Group Adult Foster Care

Group Adult Foster Care (GAFC) is a program designed for elderly or disabled individuals who need assistance with activities of daily living through regular caregiver visits.

Keeping Individuals Safe at Home and out of Long-term Care Facilities

Group Adult Foster Care (GAFC) supports individuals through visits from trained caregivers, who may be a friend, family member, or Brockton AFC staff.

Caregivers assist with personal care, supervision, and daily household activities, all while being supported by Brockton AFC.

Each client is assigned a Registered Nurse to oversee their needs and develop personalized care plans. A Case Manager provides support for caregivers, coordinates with medical and insurance personnel, advocates when needed, and connects families with resources. Bilingual services are also available.

Personal Care

Clients receive support with activities like bathing, dressing, mobility, and other daily needs.

Daily Life Activities

Caregivers visit daily to assist with tasks like grocery shopping, laundry, meal preparation, homemaking, medication reminders, and more.

Companion Care

Clients can spend time with a friend or VITRA Health-supported caregiver to promote social and mental well-being.

Assigned Care

Being a caregiver can feel like a full-time job with its own challenges and stresses. We’re here to support both you and your loved one every step of the way.

Client Eligibility
  • Need cueing or supervision with one or more Activities of Daily Living (ADLs).
  • Be at least 22 years old.
  • Have Mass Health Standard (Medicaid), Mass Health CommonHealth, or Senior Care Options coverage.
  • Live in a safe home environment.
  • Complete an annual physical exam (can be arranged with Brockton AFC)
  • Be at least 18 years old.
  • Provide daily visits to assist the person needing care.
  • Cannot be related to or legally responsible for the person receiving care.
  • Pass a CORI (Criminal Offender Record Information) check.
  • Follow the AFC Care guidelines set by the state of Massachusetts.
  • Caregiver Education: Resources and training to support caregivers.
  • Customized Care Plans: Tailored to meet the client’s unique needs.
  • Assigned Support Team: A Registered Nurse and Case Manager for ongoing assistance.
  • Stay at Home: Clients can remain in the comfort of their own home, avoiding long-term care facilities.
  • Insurance Support: Help navigating coverage and benefits.
  • Bilingual Staff: Services available in multiple languages.
  • Monthly Visits: At least one visit per month from a Nurse or Case Manager.
Steps to Approval

We guide you through every step to make the approval process as smooth as possible. Here’s what to expect:

  1. Phone Interview to Start the Admission Process

    • A care access team member will call to complete your application and answer your questions.

    • They’ll request copies of your photo ID, proof of address, and help with signing a Release of Information for your primary care provider.

    • You or your caregiver will need access to email.

  2. In-Person Assessment by a Registered Nurse and Case Manager

    • After receiving notes from your primary care provider, the care access team will schedule a home visit.

    • The registered nurse will assess if you qualify for the program through a conversation-based evaluation.

    • The case manager will ensure the home is safe, clean, and equipped with safety tools like a first aid kit, fire extinguisher, and smoke/CO2 alarms.

  3. Request Medical Documentation

    • The care access team will contact your primary care provider to collect necessary documents, including a medication list, diagnosis list, and recent office visit notes.

    • If you haven’t had a physical in the last 12 months, you’ll need to schedule one with your PCP.

  4. Submit to Insurance for Authorization

    • The care access team will send the registered nurse’s assessment and your primary care provider’s order to the insurance company for approval.

  5. Admission Visit and Custom Care Plan Creation

    • Once approved, the care access team will schedule an admission visit with the registered nurse and case manager.

    • You’ll sign an agreement with Brockton AFC, and daily care will begin right away.
