Adult Foster Care ( AFC)
Adult Foster Care (AFC) is designed for adults who need daily help with personal care due to a chronic illness, disability, or age-related challenges.
Instead of institutional care, members can live in a warm, supportive home environment — often with someone they already know and trust.

Regular visits from a registered nurse and case manager
24/7 support for urgent needs
Paid caregiver training
Financial stipends for caregivers
Group Adult Foster Care
Group Adult Foster Care (GAFC) supports individuals through visits from trained caregivers, who may be a friend, family member, or Brockton AFC staff.
Caregivers assist with personal care, supervision, and daily household activities, all while being supported by Brockton AFC.
Brockton GAFC provides:

How Brockton AFC Can Help
Adult Foster/Family Care (AFC) is a state-funded program for individuals living with a caregiver that receive physical hands-on assistance or supervision/cuing with Activities of Daily Living.
The AFC is responsible for oversight and compensating caregivers to care for individuals in a home-setting. Are you a caregiver and spend your days physically assisting or supervising with ADL’s? Brockton AFC is the solution.
